Without fear of the pandemic: activity of the donation program at the Irapuato General Hospital in Guanajuato, Mexico 2020 to 2022

Introduction. In Mexico, due to the conversion of hospitals to treat COVID-19 patients, the units with donation and/or transplantation were affected by suspending their programs, but they tried to continue the activity with the recommendations of the National Transplant Center (CENATRA) and medical evidence international available 1,2,3
Methods. A descriptive and retrospective study of the impact of the COVID-19 pandemic on the donation activity of the Irapuato General Hospital in Guanajuato, Mexico is presented.
Results. Since the declaration of a pandemic in Mexico in March 2020, 24 organ and/or tissue donations were made at our hospital until the year 2022 (Table 1), of which 71% were male and 29% female, 37 years old (±14 years) was the average age of the donors, in most cases (67%) the cause of death was head trauma, 50 organs were not extracted, mostly because they were not viable (66%), although at the beginning of the pandemic, up to 56% had no receptor available. During the second quarter of 2020, a priority corneal allocation was covered as the only donation reported in the country 4 . In all cases, the RT-PCR SARS-CoV-2 test was performed prior to organ and/or tissue removal, with an average time of 13 hours (±12 hours), improving the waiting time in the last year of 7 hours (±9 hours); The result was positive in only one donor with a cycle threshold (Ct) of 34, accepting liver extraction to cover a National Emergency, being the first case reported in Mexico. No suspected cases of COVID-19 infection in organ and/or tissue recipients attributed to donors were reported by transplant hospitals.
Conclusions. The Irapuato General Hospital, like others nationally and internationally in the donation activity during the pandemic, has considered the risk of mortality of recipients on the waiting list compared to the risk of transmission of the recipient and the transplant team before the donation processes 5. Through these results, we reflect the impact of the pandemic on donation activity, with less waiting time for the development of the processes, in addition to setting an antecedent and reference in the medical evidence of our country.
